ScienceDaily (Nov. 6, 2012) ? Scientists have identified a group of small molecules that interfere with the activity of a compound that initiates multiple steps in blood clotting, including those that lead to the obstruction of veins or arteries, a condition called thrombosis. Blocking the activity of this compound, polyphosphate, could treat thrombosis with fewer bleeding side effects than the drugs that are currently on the market.
Their findings appear in the journal Blood.
Blood clots are formed at the site of an injured blood vessel to prevent blood loss. Sometimes, however, blood clots completely clog an artery or vein and the surrounding tissues are damaged. The U.S. Centers for Disease Control and Prevention reports that annually, 300,000 to 600,000 Americans are afflicted with deep vein thrombosis or pulmonary embolism, a blocked lung artery that often results from thrombosis, and 60,000 to 100,000 people die each year as a result of these conditions.
There are two pathways that trigger blood clotting. The tissue factor pathway helps stop bleeding if a person is injured. If any of the proteins of this pathway are missing, a bleeding problem will develop. In contrast, the contact pathway is activated when blood comes into contact with some artificial substances. Although this pathway can cause pathological blood clots, humans who lack proteins in this pathway do not have bleeding problems. These two pathways eventually converge to form a common pathway.
In 2006, the researchers found that compounds called polyphosphates can, when released from cell fragments called platelets, activate the contact pathway, said University of Illinois biochemistry professor James H. Morrissey, who led that study and the new analysis.
Because the contact pathway is not essential for normal blood clotting after an injury, interrupting polyphosphate "wouldn't have the bleeding side effects that touching anything downstream of it in the clotting cascade (would) have," Morrissey said.
The researchers found a variety of positively charged molecules that can bind to the negatively charged polyphosphate molecule and inhibit its ability to induce blood clotting. By adding these compounds to human blood and plasma isolated from the body, Morrissey and his colleagues were able to measure their effectiveness in inhibiting polyphosphate's pro-thrombotic and pro-inflammatory activities.
The researchers also tested these inhibitors in mice that were afflicted with venous and arterial thrombosis or inflammation, and found that these inhibitors prevented or reduced these negative effects.
"What this shows is that you could put really potent inhibitors of polyphosphate in and interrupt the clotting system by decreasing thrombotic risk, but probably not increasing (a person's) bleeding risk," Morrissey said. "This is the proof of principle that it works."
Although the compounds identified would not, by themselves, be good drug candidates, Morrissey said, the new study offers clues for developing more suitable drugs to target polyphosphates.
"I think that the work going forward would be to identify compounds that would be better drug candidates," he said.
The study team also included researchers from the Medical College of Wisconsin. The National Heart, Lung and Blood Institute at the National Institutes of Health supported this research.

Every year, I have thousands of interactions with readers, friends, and other bloggers about personal finance. Over the years, I think the thing that has scared me most is the potential for a major illness or injury to wipe out your finances.
Think about it: One day you?re on your way to work, the next day you wake up in the hospital after a terrible car accident and learn you need three surgeries and two years of physical therapy to recover.?Not only will such an injury take a massive toll on your body and your life ? there?s that not-so-little issue that we live in the United States. Health care isn?t free.
A large number of personal bankruptcies result from medical bills that pile up after an illness or an injury.
Although the uninsured are at greatest risk for facing unaffordable healthcare bills, even if you have insurance with a high deductible and out-of-pocket maximum, a major illness could take a toll.
This brings up two points:
You may not be able to control your health or avoid an accident, but you can be savvy about coping with medical bills. Here are some tips.
The first thing to do when you start to get medical bills is get organized. For example, having a baby is anything but free, so as the doctor?s bills from Lauren?s pregnancies began to roll in, we scanned in all of her bills and the explanations of benefits (EOB) ? the records your insurance company sends to show what they paid and what you owe. I also created a spreadsheet to track everything both the insurance company and we have paid, so we will know when we hit our deductible and out-of-pocket max.

It?s grossly unfair, but the rates medical insurers pay for an ER visit, blood test, or doctor?s appointment may be as little as a third what an uninsured patient will have to pay. That?s because insurers pre-negotiate rates based on the millions of patients in their network.
That?s the bad news. The good news is: You can negotiate your medical bills, too. You can either attempt to negotiate directly with the hospital and doctor?s office or you can even find professional healthcare negotiation consultants who will work to reduce your bills for a fee.
When faced with thousands in medical bills that you can?t pay, it may be tempting to put them all on a credit card and worry about it later. (After all, almost every medical biller happily accepts MasterCard or Visa.) But unless you have a surefire plan for repaying the entire balance in short order, that may just be robbing Peter to pay Paul. And Peter?s going to charge interest!
Secondly, do NOT borrow against your home to pay medical bills. Medical bills are unsecured, meaning if you can?t pay them, the hospital doesn?t have a legal claim to any of your property. A home equity loan, however, is secured by your house, meaning if you don?t pay, the bank can foreclose on your home.
If you can?t afford big medical bills, you may be able to ask for a payment plan with the billers, which would be preferable to borrowing.
If you don?t pay your medical bills, they will eventually be sold to collection companies who will hound you for payment. These collections accounts may also appear on your credit report as negative items. The upside, however, is that you may be able to negotiate with the collection agencies to settle for a less than the full amount. If you do, just be sure to get the agreed settlement in writing before sending payment.

I?m an advocate of accepting responsibility for your debts and paying them in full, but if you?re over your head because you got sick ? or perhaps you can no longer work because of medical issues ? bankruptcy is there as a lifeline.
Depending on your income, medical bills can be discharged in Chapter 7 bankruptcy or put into a court-administered payment plan that you can afford under Chapter 13 bankruptcy. In either case, bankruptcy is not a decision to be taken lightly and should only be made after you?ve exhausted alternatives and consulted with an attorney.
Although you can?t plan for every contingency, the best way to protect yourself from getting in over your head with medical bills is having health insurance and having savings. If you don?t have one ? you most definitely need the other!

ScienceDaily (Nov. 3, 2012) ? A drug therapy shows promise for treating an inherited form of kidney disease called autosomal dominant polycystic kidney disease (ADPKD), Mayo Clinic researchers say. The medication, tolvaptan, slowed the pace of kidney cyst growth over the three years of the study.
The phase three clinical trial results were being presented November 3 at the American Society of Nephrology annual meeting and published online in the New England Journal of Medicine.
The multicenter study found tolvaptan demonstrated a nearly 50 percent reduction in the rate of increase in total kidney volume (a measurement of kidney cyst growth) in ADPKD patients over the study period, compared to placebo.
"ADPKD is the most common inherited and the fourth most common overall cause of kidney failure worldwide," says lead author Vicente Torres, M.D., Ph.D., Mayo Clinic nephrologist.
"In most patients with this disease, relentless cyst growth within the kidneys destroys the tissue, causes hypertension and painful complications, and negatively impacts the quality of life," Dr. Torres says. "The results of this study reveal a potential treatment that blunts kidney growth, lessens associated symptoms and slows kidney function decline when given over three years."
While the trial findings are encouraging, tolvaptan has not yet been approved for this indication, Dr. Torres notes.
The study was funded by Otsuka Pharmaceuticals.
